This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

The short version. This notice is about your protected health information — the record a clinician builds when they care for you. It says who may see it, when we need your written permission, what rights you have over it, and how to complain if you think they have been ignored.

This is a HIPAA notice of privacy practices and it is a regulated document with prescribed content. It has to be reviewed by privacy counsel, and the covered entity, effective date and complaint contact have to be filled in, before this page is published. The standfirst above is the statutory header statement, set in sentence case to match the brand rather than in capitals as the regulation prints it — counsel should confirm that is acceptable.

Who this notice covers

This notice applies to [covered entity or organized health care arrangement to be confirmed] and to the clinicians who care for you through the dose platform.

dose acts as a business associate where it handles protected health information on their behalf, and is bound by a written agreement to protect it on the same terms.

Our duties

We are required by law to keep your protected health information private, to give you this notice of our legal duties and privacy practices, and to follow the notice that is currently in effect.

We are also required to tell you if a breach affects the privacy or security of your information.

How we use and disclose your information

For treatment: so a clinician can review your case, prescribe if appropriate, and coordinate with a pharmacy or another clinician involved in your care.

For payment: so a review or a prescription can be billed and paid for. For health care operations: so care can be reviewed for quality, so staff can be trained, and so the service can be run and audited.

Disclosures that do not need your authorization

[To be completed by counsel.] The law permits certain disclosures without your permission — for example when required by law, for public health activities, to report suspected abuse or neglect, for health oversight, for judicial proceedings, to law enforcement in defined circumstances, to avert a serious threat to health or safety, and for workers’ compensation.

Each category has conditions and limits, and this section has to state them rather than list the headings.

Disclosures that need your written authorization

Most marketing, any sale of your protected health information, and the use of psychotherapy notes require your written authorization before we may proceed.

You may revoke an authorization in writing at any time. Revoking it stops future use; it does not undo what was already done while it was in force.

Your rights

You have the right to see and get a copy of your record, in an electronic form if you ask for one; to ask us to correct something you believe is wrong; to ask for a list of certain disclosures we have made; to ask us to limit what we use or share; to ask us to contact you in a particular way or at a particular address; to a paper copy of this notice; and to be told about a breach.

We must act on most of these requests, and we must tell you in writing if we decline one and why.

How to use your rights

Write to the contact at the foot of this page and say which right you are using and what you want to happen. We will confirm that we have your request and tell you how long it will take.

There is no charge for making a request. A reasonable, cost-based fee may apply to some copies, and we will tell you the amount before we produce them.

How to complain

If you think your privacy rights have been ignored, write to us first — we would rather hear it from you and put it right.

You may also complain to the U.S. Department of Health and Human Services, Office for Civil Rights, at 200 Independence Avenue SW, Washington, D.C. 20201, by calling 1-877-696-6775, or at hhs.gov/ocr/privacy/hipaa/complaints. We will not retaliate against you for complaining.

Changes to this notice

We may change this notice and apply the new terms to information we already hold. The version in force is always the one on this page, and the effective date at the top says when it began.

If we make a material change, we will post the new notice here and make it available on request.
